Why keys could not be prepared for an identifier, on an identifier.prepare record with outcome denied:

• agent-refused: the agent the organisation is placed on would not generate the keys. Nothing was generated that the identifier holds, and no document was rendered. The caller is answered identifier.agent_refused.

An agent that did not answer is not a denial and writes no record: the step is retried under the same message id (identifier.agent_unavailable). The list only widens.
