bootstrap.plan: the setup engine ran in plan mode (D3.11) and changed nothing. wouldRefuse says whether an apply of the same answers would have been refused at some step, so a plan that found a refusal leaves it on the trail. Optional only because a required member is not additive; the one producer always sets it.
